What should we bring from the hospital?

Please bring the discharge summary, current prescriptions, reports, mobility aids and any written instructions for diet or wound care.

Is this a replacement for a hospital?

No. Gracias Living provides residential support after discharge. A person who needs hospital monitoring or emergency treatment should remain under hospital care.
